Please note, the vaccine will be administrated in two doses.
For your first appointment, please be sure to bring your California identification card and one of the following: The State of California Child Care Site License, Employee Identification Badge, paycheck stub, or an official letter verifying one of the following listed above.
After receiving your vaccination, you will sit in the observation area for 15-30 minutes. Then, you will receive a vaccination card with your second appointment date of April 20 (21 days later) at the same location.
